	Please spell patient names or uncommon proper nouns.
	Please spell referring / consulting physician names (unless referring physician list is on file with us).
	If you require changes to your regular templates, please dictate detailed instructions.
	“If we cannot hear it, we cannot transcribe it”. Therefore, please do not dictate in an overly noisy environment and please do not mumble or skip words.
	If you are unsure about the template (or work-type) number to use, please dictate which template (or work-type) we should use.
